Boise School District Screenings

Description:
The District and St. Luke’s Occupational Health Clinic screening includes biometric measures for blood pressure, resting pulse, height, weight and waist circumference as well as laboratory tests for cholesterol, triglycerides, HDL, LDL and glucose. Results will be mailed to your home address along with instructions for completing the online Personal Wellness Profile (PWP). Boise School District has identified health screening and the PWP as requirements for the 2011/12 Wellness Plan.

IMPORTANT! PLEASE READ:

  • Remember to FAST – no food or drink except water – for 8 to 12 hours prior to your appointment. You may take your prescription medications as usual.
  • Additional labs are available with a doctor's order. Download the Lab Request Form at www.boiseschools.org/benefits/wellness and have your primary care physician complete it. Please bring the form along with your insurance card to your screening appointment.
