What is Family Medicine?
Family Medicine doctors are trained to care for patients of all ages, from infancy through the end of life. We promote the concept of a Medical Home, emphasizing a continuous relationship with a personal physician coordinating care for both wellness and illness. We believe that knowing you over many years, and knowing your family, helps us understand how your health care impacts your life.
St. Luke’s Family Medicine physicians diagnose and treat most medical issues, from infections and sprains to diabetes and asthma. We also specialize in preventive care to help you create and maintain a healthy lifestyle. And when your condition requires a specialist, we can coordinate your care with other physicians and can help you navigate your way through today’s health care.

Sean Hackett, PA serves as both clinical manager and a practicing physician assistant. He earned his bachelor’s degree from Boise State University and his master’s degree in molecular biology from the University of Idaho. He earned his physician assistant degree from Idaho State University. Sean has been practicing in Fruitland for 14 years. |

Nicole Hoyt, PA-C graduated from Northwest Nazarene University in Nampa in 2005 with a Bachelor of Science degree in Biology. She earned her Master of Science degree in medicine at Trevecca Nazarene University in Nashville in 2007. Nicole has received her certification as a physician assistant by the National Commission on Certification of Physician Assistants. |

Sean Page, PA graduated from Idaho State University in 2003 with a Bachelor of Science degree in clinical laboratory science. He earned his Master of Medical Science in physician assistant studies from Midwestern University in Glendale, Arizona in 2009. Since returning to Idaho he has been working in family practice and urgent care. |

George Roth, PA served as medical corpsman with the U.S. Air Force before earning his Bachelor of Science degree and physician assistant certificate from Wichita State University in Wichita, Kansas 1990. He earned his masters degree in physician assistant studies from the University of Nebraska and has worked emergency, urgent care, and family practice medicine. |
Your Appointment
We try to stay on time, as a courtesy to you and others. Since paperwork may take a few minutes, please arrive early. As emergencies do occur, there may be occasions when your appointment time will be delayed. We will always try to keep you informed in the event of a delay, and will be happy to reschedule your appointment, if you prefer. We ask that you respect the appointment time we have reserved for you, and call 24 hours ahead to cancel or change appointments so that others may be helped during that time.
Communication with your provider is extremely important. It may be helpful for you to write down questions for your doctor or nurse practitioner prior to your visit. Bring a list of medications and the dosages, or bring the bottles with you.
Make sure you understand any instructions you are given. If clarification is needed, review your instructions with the provider or medical assistant before you leave. If you aren’t doing well after your visit, we want to hear from you. Our goal is to improve your health.
Insurance Plans
We accept most insurance plans, including Medicare, in-state Medicaid, Tricare, Blue Cross/Blue Shield, and others.
Medication Refills
Talk with your provider at each visit to be sure your prescriptions are up to date. For refills, contact your pharmacy. Your pharmacy will then contact our office for verification and approval. For patient safety, narcotics cannot be refilled outside of office hours. Talk with your provider before your prescription runs out, and please allow 48 hours for refills.
