Diabetes and Nutrition Services - (outpatient)
Educational services are available for patients
with Type I, Type II, or Gestational Diabetes. The program
is provided for patients with newly diagnosed or uncontrolled
diabetes. As a part of the diabetes education, nutritional
instruction will also be provided.
An individual education plan is developed with the patient
and diabetes education team – including a Nurse Certified
Diabetes Educator and Registered & Licensed Dietitian.
This team works with the patient and physician to set behavior
goals and treat the disease. The Educator communicates and
coordinates care with the physician as needed. Patients are
typically scheduled for 3 visits, the first with the Dietitian
and the last two with the Diabetes Educator.
